Please answer all questions for the Committee to determine whether you qualify for reinstatement.

To ensure a smoother review process, be as thorough as possible with your explanations and documentation.

Certified Member $275 Annually

A Certified Member is a CSA (Certified Surgical Assistant) who successfully completes and passes the NCCSA certification examination and is in good standing with the NSAA. Annual dues for Certified Members is $300.

Affiliate Member $200 Annually

An Affiliate Member is any person who holds a surgical assistant credential other than CSA, medical organizations, medical businesses, and is in good standing with the NSAA. Annual dues for Affiliate Members is $200.

Student Member $25 One-Time Fee

A Student Member is an individual who is currently enrolled in a CAAHEP Accredited Surgical Assistant Educational Program and in good standing with the NSAA. An NSAA Student Member must become registered within 30 days of their surgical assistant program start date. A one time dues payment is required for Student Membership.
